Senate Bill 12 Required Consent Forms

Due to a new Texas law, for the 2025-2026 school year, SB 12 requires parents to sign consent forms for services that have previously been provided to students. This includes Health-Related Services, Counseling Services, and Club participation.
This legislation mandates that parents/guardians are fully informed and give permission for care and support. Without your consent, nurses, counselors, athletic trainers and other staff will not be able to provide certain services.
In a serious emergency, staff will still act to protect your child’s safety and will contact you immediately.
Parents/guardians should complete the consent for School Health- Related Services Form, Consent for School Counseling Service Form, and Consent to participate in Student Clubs as soon as possible.

Debido a una nueva ley de Texas, para el año escolar 2025-2026, la SB 12 exige que los padres firmen formularios de consentimiento para los servicios que se hayan brindado previamente a los estudiantes. Esto incluye servicios relacionados con la salud, servicios de consejería y participación en clubes.

Consent for School Counseling Services |
Examples of mental health services provided on campus may include, but are not limited to:
- Individual and group counseling
- Emotional behavioral support, social skill development, and a student-needs assessment survey
- Access to trusted adults, and identify potential topics for target instruction or support
- Support during emotional distress or crisis: While staff can take immediate action in a life-threatening emergency (e.g., a student at risk of self-harm), routine support during emotional distress or a crisis requires parental consent.
- In a life-threatening emergency, staff will respond immediately and notify parents right away.
- If the form is not completed, parents will be fully responsible for their child’s physical and mental health care outside of emergencies.

Consent for Health-Related Services |
Examples of health-related services provided on campus may include, but are not limited to:
- Injury evaluation and first aid care
- State-mandated health screenings (vision, hearing, spinal, acanthosis)
- Lice screening
- Assessment and care for illness or physical symptoms while at school
- Coordination of health services
- Heat illness prevention and injury support for student athletes
- Hygiene support

Consent to Participate in Student Clubs |
The District must receive written parental consent before a student may participate in any student clubs
authorized or sponsored by the District or campus. Only clubs authorized or sponsored in compliance with state law will be available for participation.

You must complete a School Health Services Consent Form for each child in United ISD.

Without the consent form, school staff—including nurses and counselors—cannot provide your child with routine physical or mental health services. You will be responsible for arranging all care for your child outside of emergencies.

Examples of physical health services include:
- First aid and illness care at school
- Vision, hearing, spinal, and other state-required screenings
- Lice checks
- Injury and heat illness support for athletes

This form does not include:
- Prescription or over-the-counter medication administration
- Ongoing treatment for chronic health conditions
- Formal medical or psychological diagnoses
These require separate written parent permission.

In a life-threatening emergency, school staff will respond immediately to protect your child’s safety and contact you right away.

Consent for School Counseling Services That Support Mental Health FAQs

Examples of services by school counselors or mental health staff that may be provided with parental consent are:
- Short-term individual or small group counseling for emotional or behavioral support
- Social-emotional learning
- Support during emotional distress or crisis

No. Parental consent is not required for school counselors to provide academic services. Counselors will continue to support students with:
- Course selection and scheduling
- College and career readiness planning
- Academic goal setting and monitoring
- General school success strategies
Note: This law (SB 12) only applies to mental and behavioral health-related counseling services, not academic counseling.

Parental consent is not required for these services:
- Medical emergencies or those required by law including:
- Immediate help if a student is at risk of self-harm or experiencing suicidal thoughts
- Support for grief after a death or traumatic event
- Situations involving suspected abuse or neglect (required by law to report)
- Other urgent matters required by state or federal law
Note: Under Texas law, a child may also consent to their own counseling in specific cases, including:
- Suicide prevention
- Chemical addiction or dependency
- Sexual, physical, or emotional abuse

School counselors do not provide some services and would require separate parental consent through outside providers, such as:
- Medical or psychological diagnoses
- Long-term therapy, counseling, or treatment
Note: UISD counseling staff do not provide medical or psychological diagnoses, long-term therapy, or treatment. When students need these types of services, families are connected with community agencies that require their own parental consent.

- School Counselors and Responsive Services Counselors provide short-term, school-based counseling and crisis support.
- Teachers and staff lead short SEL classroom activities that help students feel safe, connected, and ready to learn. These activities are not counseling or therapy.
