Self-Contained Camera Incident Video Footage Request Form

School Year Camera Time Periods and End Dates:
- Regular school year:
- August-May
- Summer (ESY) school:
- June-July
Activation Form Request to be submitted to the campus principal
- English
- Spanish
- Campus Admin. email addresses
- Once both signatures have been completed on the form, please email the document to:
- spedcamerainstallationrequest@uisd.net
Important Timelines:
- Video Footage Requests: Seven days to complete the request, from the date on the request form
- Sp. Ed. Camera Installation Requests: 10 - 45 days to complete the request, from the date on the request form
- Sp. Ed. Self-Contained Recorded Footage on File - Three months back from the date requested on the form
Note: Sp. Ed. Camera DOES NOT follow the student. If the student moves to another self-contained unit (even if it's on the same campus), the requester MUST submit a NEW request to the campus administrator.
